Denise Ann Jack, 62, of Fairmont, passed away May 28, 2016, at the United Hospital Center.
Denise was born May 20, 1954, in Fairmont, daughter of the late Ray Sloan Sr. and Louise Aretta Hawkins Sloan.
Denise was a member of the New Life Ministries and a former girls' softball coach. She loved to show horses and enjoyed fishing. She was a former employee of Westinghouse Corp.
Survivors include her husband, Donald Jack; a son, Mark Jack and wife Brittney of Fairmont; sisters, Barbara Floyd and husband Larry of Barrackville and Sandra Echols of Christianburg, Virginia; a brother, Ray Sloan Jr. and wife Linda of Fairmont; four grandchildren, Mayson, Slayton, BryLeigh and Jaysea; sister-in-law, Bilinda Gearde; brother-in-law, Robert Jack; five nephews, Tom Sloan and wife Jenny, Mike Sloan and wife Karla, Nicholas Gearde, Matthew Jack and Rick Floyd and his wife Rachael; two nieces, Leigh Ann Vingle and Laila Jenkins; three great-nephews, Justin and Ethan Sloan, and Easton Gearde; and five great-nieces, Courtney, Alyssa Sloan, Jocelyn Jenkins, Roselyn and Emma Floyd.
The family will receive friends from 2-8 p.m. Tuesday and from 10-11 a.m. Wednesday at Domico Funeral Home 414 Gaston Ave. in Fairmont. Funeral service will be at 11 a.m. in the funeral home with Pastor Charlie Chipps officiating. Burial will follow at the Grandview Memorial Gardens Cemetery.
